� To take history of all patients unbiased taking into account whole of the patients.
o To take Complete HISTORY on first visit inclusive of Past History, Family History, Personal History, Drug History, Allergies, Menstrual & Obstetric Histories of Female.
o To perform Complete Physical examination [General, & Systemic including Neurological examination].
� Special emphasis will be given for the following;
o Diabetic Patients (on first visit):
? History;
� Documentation of complete history in the printed form.
� Previous blood glucose monitoring.
� Current treatment
? General & Physical Examination;
� Blood Pressure, Pulse.
� Weight & Height.
� Foot Examination (sensation, pulses and reflexes)
? Visual Acuity;
? Instillation of eye drops for Fundoscopy.
? Laboratory Investigations (especially HbA1C, Creatinine, Urine D/R)
o Diabetic Patients (on follow-up visit):
? Presenting complaints (reason for the visit)
? Record of SMBG
? Current treatment [ placing the last prescription as the first page of file ]
? General physical examination (B.P, Pulse, Weight, Height)
? Labs if needed.
o Diabetic Patients (on annual review visit):
? Documentation in the new form.
? Presenting complaints (reason for the visit)
? General physical examination (B.P, Pulse, Weight, Height)
? Foot Examination (sensation, pulses and reflexes)
? Record of SMBG.
? Visual Acuity.
? Instillation of eye drops for Fundoscopy.
? Laboratory Investigations (especiallyHbA1C, Creatinine, Urine D/R)
? Current treatment [ placing the last prescription, as the first page of file ]
o General Medicine / Endocrine Patients
? Documentation on the Card.
? Complete history.
? General physical examination (B.P, Pulse, Weight, Height).
? Systemic examination (respiratory & cardiovascular, abdomen etc.) more in detail according to patient’s complaints.
? Foot Examination, examination of thyroid, Hirsutism score, Acanthosis Nigricans, Visual Acuity (in relation of patient’s complaint)
? Pertinent Laboratory Investigations
? Current treatment [ placing the last prescription as the first page of file
� To attend Emergency Call, assess and carry our urgent tests and inform
Registrar/Consultant on Call.
� To admit the patient following all protocol through Reception.
� To keep consultant on call abreast of all admission from OPD.
� To be present with patient undergoing procedure in OPD.
� To facilitate arrangements for transfer of patient to Emergency or Ward.
� To guide and teach medical students / diploma students and House Officers, if posted.
� To perform duty in WARD as and when required during day or night to facilitate the smooth working of the WARD.
� Perform minor procedures under the direction of consultant.
� To maintain Patient Assessment electronically with the Help of HCMS
